A one-time fee of $50 for the Virtual Mastercard Pro, $100 for the Physical Visa Pro or $250 for the Physical Visa Ultimate. On top of that: a 2.6% top-up fee with Gold status, and a transaction fee – $0.50 per payment on the Virtual Mastercard Pro, $0.10 only on payments under $1 on the Physical Visa Pro, and $0.10 to $0.50 depending on the amount on the Physical Visa Ultimate.

Yes, with either physical card. Each withdrawal costs 3% (minimum $1). Physical Visa Pro: up to $1,500 per withdrawal, $3,000 per day, $10,000 per month and $100,000 per year, with at most 3 withdrawals a day. Physical Visa Ultimate: up to $1,500 per withdrawal and $15,000 per month.
